Please bring a completed organizer along with all original supporting documents you have received with you to your appointment, or mail or upload them to me (see the "Send A File" tab).  Do not enter information in more than one place in the organizer, and do not make and entry for any information for which you have a document (ie W-2, 1099, 1098, etc).  Returning clients do not have to complete page 1 unless any information has changed.

If you are a first-year client, or you have a new license or SS Card, I will need copies of Drivers Licenses for you and your spouse, and Social Security Cards for you, your spouse and your dependents (no exceptions), as well as copies of your 2021 and 2022 federal and state tax return(s) as filed.
